Essential to the CMS approved approach of reducing health disparities and addressing the social determinants of health is the planned award to up to 13 entities across nine NY regions to become NYS-designated Social Care Network leads, which will allocate $500 million in SDOH Infrastructure investments.
These lead entities will play a pivotal role in coordinating the delivery of $3.1 billion in reimbursable social care services across a network of community based organizations to Medicaid members, bridging crucial gaps in healthcare access and fostering community well-being.
